The Hospital CDI Readiness Checklist: 10 Questions to Benchmark Your Program
Updated: Oct 3

A practical team discussion guide for CDI, HIM, clinical coding, and revenue-cycle leaders. Use these questions to spot where workflows are clear—and where one small improvement could make work easier for clinicians and reviewers.
THE 10-QUESTION CHECKLIST
1. Are CDI goals connected to organizational priorities?
Make sure program goals are clearly defined and reflect the organization’s quality, documentation, and operational priorities.
2. Are reviewer workflows and prioritization rules consistent?
Document how cases are assigned, prioritized, escalated, and handed off so teams can apply the process consistently.
3. Are clinician clarification queries clear and governed?
Review whether queries are timely, concise, non-leading, and handled under current organizational policy and applicable guidance.
4. Do you monitor query appropriateness and response patterns?
Use routine review to understand response trends, identify workflow friction, and address concerns through established governance.
5. Are coding and CDI handoffs easy to follow?
Name the owners, escalation paths, and next steps for cases that need collaboration across teams.
6. Are complex cases prioritized using transparent criteria?
Use approved, documented criteria for prioritization, and periodically confirm the criteria are applied consistently.
7. Do documentation trends lead to constructive feedback?
Review patterns across service lines and share observations in a timely, collaborative way with the right teams.
8. Is education tied to actual review findings?
Connect education topics to recurring audit, quality, or workflow findings, then revisit whether the learning need has changed.
9. Does leadership see a balanced set of measures?
Agree on measure definitions and review quality, timeliness, provider experience, and denial trends together, with context.
10. Are privacy and audit practices built into the workflow?
Confirm privacy, access, retention, and audit-trail practices follow organizational policy. Never use patient identifiers in public forms or marketing conversations.
HOW TO USE THIS CHECKLIST
Bring the questions to a CDI team huddle or leadership review. Mark each item “clear,” “needs discussion,” or “next step,” then choose one practical improvement to test. Assign an owner and a review date so the conversation turns into action.
