Demystifying the ED CDI Program: What You Need to Know!

In healthcare management and hospital strategy, the rule of proportion is

significant in hospital revenue management. However, amidst the focus on

financial management, one often tends to overlook the indispensable

contribution of the emergency medicine department to the entire hospital operations. This department is the first line of defense in managing short stays and urgent admissions. Its efficient functioning has the potential to propel the hospital toward unprecedented heights. At the same time, its suboptimal performance can prove to be the Achilles heel that causes the hospital's finances to plummet.


Like an aircraft's engine, the emergency medicine department requires

diligent maintenance to prevent resource depletion and unnecessary

turbulence in the hospital's economy. Thus, it is imperative to watch this

department closely and ensure its optimal functioning.


A careful examination of the ED problem list is crucial to handling the influx

of emergency department visits, the high number of observation stays, and

the need for inpatient admissions. The underlying causes of the

challenges must be well-understood and supported to prevent their

persistence. Simultaneously, this examination must be performed on a

large scale to ensure that the emergency medicine department's

challenges are mitigated effectively.


Importance of Clinical Documentation Integrity (CDI) in Emergency

Just like a flight can't take off without a cockpit, hospitals can't survive without their forefront, the ED.

If a flight crew is extremely good to their customers, but the plane runs out of gas, the story ends in tragedy. Similarly, hospitals may provide quality care to their patients, but the end game can be catastrophic if proper Clinical Documentation doesn't support ED levels of care or inpatient admissions.

  1. Diagnoses, Present on Admission POA:


Diagnoses Present on Admission (POA) refers to medical conditions or illnesses documented as being present when a patient is admitted to a hospital or healthcare facility. These diagnoses are essential because they help healthcare providers distinguish between conditions that developed during the patient's hospital stay and those that were pre-existing. Identifying POA diagnoses is crucial for accurate billing, appropriate treatment planning, and ensuring transparency in healthcare outcomes.


  1. ED importance of levels of service dissection:

In the Emergency Department (ED), service levels refer to categorizing patient visits based on the complexity and intensity of care required. Typically, EDs use a tiered system, such as the Emergency Severity Index (ESI), to prioritize patients and allocate resources effectively. The levels range from Level 1 (most severe) to Level 5 (least severe), with Level 1 patients requiring immediate life-saving intervention and Level 5 patients needing minimal intervention or assessment. Dissecting these levels helps ED staff triage patients efficiently, manage resources, and provide timely care based on individual needs and understanding.


3. MDM vs. Medical Necessity: Training and vision are needed to differentiate between ED medical decision-making (MDM) and medical necessity.


Medical Decision-Making (MDM): MDM refers to the process through which healthcare providers assess and determine the appropriate course of action for diagnosing, treating, and managing a patient's condition. It involves evaluating the patient's history, symptoms, diagnostic tests, and treatment options to decide on the best course of action.


Medical Necessity: Medical necessity refers to the requirement that healthcare services and treatments provided to patients must be reasonable and necessary for diagnosing or treating their medical condition. In other words, medical services or procedures must be justified by the patient's medical condition and be consistent with accepted standards of medical practice. Insurance companies often use medical necessity criteria to determine whether they will cover the costs of certain medical services or treatments.

This Triad (POA Diagnoses, Levels of care, MDM vs Medical necessity) must align and be supported by proper staffing and technology to help ED departments stratify, study, and report outcomes, revenue, population mix, and patient satisfaction. Remember: if an event or action is not documented, it can be difficult to prove that it took place. Especially in this fast-paced environment, the ED.


Timing of Clinical Documentation Integrity CDI:


The current state of the healthcare system presents a significant challenge for healthcare providers and Hospitals. With health insurers, such as the Centers for Medicare and Medicaid Services (CMS), increasingly scrutinizing medical necessity for select health services, healthcare providers must explore how the time-lapse of services provided impacts their patient outcomes.


Research has shown that providing care to patients in ED observation units (OU) can be a viable alternative to inpatient admission, resulting in better clinical and perhaps economic outcomes. However, monitoring hospital resource consumption closely during this process is essential.

To better understand, let's think of the plane analogy again. Just like the body of an airplane represents its overhead and operations, the utilization of hospital resources during an ED visit is defined by the belly of the plane. If the use of resources spirals out of control during the ED visit, the belly of the aircraft will grow too big, and the plane will crash. To ensure that every ED visit, Observation care, or Observation to inpatient admission is economical and productive, hospitals must prioritize a productivity playbook, ensuring every team member contributes to the priorities of economic and patient outcomes. ED CDI provides a thorough and precise framework for this playbook, covering all necessary details at scale and in a timely manner.


Implementation Strategies for Clinical Documentation Integrity (CDI):

Gain control: Similar to an aircraft, hospitals require a balanced strategy to further propel their engines and ensure revenue and data integrity. This balance can be achieved by taking various factors into consideration.

Education and training programs can equip healthcare providers with the necessary knowledge and skills to document accurately and efficiently. Also, differentiate between medical necessity and medical decision-making. Integrating CDI best practices into electronic health record (EHR) systems can streamline documentation workflows and provide real-time prompts and validation for the problem list. It promotes a shared responsibility for maintaining data integrity among team members treating the same patients.
