About the Role

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team for a Per Diem position at a skilled nursing facility in Chula Vista, CA. As an LVN, you will play a crucial role in providing high-quality patient care and ensuring the well-being of the patients in the home. AM Shift: 7:00 AM - 3:30 PM

PM Shift: 3:00 PM - 11:30 PM

Nocturnal Shift: 11:00 PM - 7:30 AM

Requirements

  • Valid license as LVN issued by the California State Board of Vocational Nurse Examiners

  • Minimum of six (6) months of experience within the last three (3) years rendering direct patient care in public or private institutions

  • Clinical competencies must be current

  • Minimum of 6 months of experience working with an electronic medication administration record system and an electronic medical records system

  • Intermediate MS Office skills (Word, Excel, Outlook)

About the Company

Organize, provide, and maintain necessary health care services consistent with CalVet policies and procedures

Plan and implement nursing care. This includes administration of medication, therapeutic agents, treatments, disease prevention, physicians’ orders, and obtaining specimens for diagnostic testing.

Participate as a member of the interdisciplinary team in developing multi-disciplinary treatment efforts; complete assigned weekly summaries, update care plans, assist physicians with rounds; note and institute physician orders

Collect information regarding patient status and report the results to the Primary Care Staff (PCP) or RN in a timely manner

Provide Resident education and accurately document care in the approved format

Assist mental health professionals by contributing to the treatment planning process and the formal evaluation of Resident progress by monitoring and recording observed patient behavior

Document in the Resident’s health record the nursing care and treatment provided and the Resident’s response to facilitate the continuity of care and communicate to other health care Staff

Ensure that staff utilizes optimum standards of resident care and that staff members observe safety and Infection Control Precautions and regulations at all times.
